What Is a Cardiac Stress Test?

A cardiac stress test is a diagnostic exam used to analyze how the heart responds to exertion. The patient is instructed to exercise, forcing their heart to work harder. Electrocardiogram (EKG) readings are taken before and after exercise to measure the change in heart rate and blood flow. The results may be used for several applications, including:

  • Diagnosing heart disease and arrhythmias
  • Predicting a patient’s risk of heart attack
  • Planning treatments for heart disorders
  • Evaluating the effectiveness of treatment

Cardiac stress tests are sometimes used by themselves. However, they are often administered alongside other non-invasive outpatient cardiology tests. By using multiple tests, physicians can form a clearer picture of a patient’s condition for a more informed response.

What to Expect on Testing Day

Before a cardiac stress test, the cardiologist will provide the patient with clear instructions on how to prepare. Patients are often asked to avoid eating, drinking, and smoking on the day of the test. They may also need to pause certain medications. Patients should wear clothes and shoes that are comfortable for exercising and should bring an inhaler if they use one.

The test procedure follows these steps:

  1. Several sticky patches, called electrodes, are attached to the patient’s arms, legs, and torso. The electrodes measure the electrical impulses that accompany the heart rate.
  2. A cuff is placed around the patient’s arm to measure blood pressure.
  3. Under the observation of a technician or cardiologist, the patient begins exercising on a treadmill or stationary bicycle. Exercise begins slowly and becomes more intense as the test progresses.
  4. The patient may then be asked to sit still or lie down after exercise for further monitoring.

The length and intensity of testing depend on the patient’s physical condition and the results needed by the physician. It typically takes between 15 to 20 minutes. Afterward, the patient is free to return to normal daily activities.

When to Get a Stress Test

Some heart conditions cause few problems when the body is at rest. This can present a challenge to diagnosis. Cardiac stress tests are used to evaluate cardiovascular conditions that are only evident when exercising or exacerbated by physical exertion. These conditions often cause the following symptoms, especially during periods of strenuous exercise or activity:

  • Chest pain
  • Shortness of breath
  • Dizziness or fainting
  • Heart palpitations

Cardiac stress tests may also be requested to evaluate a patient’s risk of heart attack. Certain health and lifestyle factors can increase a patient’s likelihood of developing heart disease. These include:

  • Being older than age 60
  • Having diabetes
  • Being obese
  • Being physically inactive
  • Smoking
  • Using alcohol heavily
  • Having high cholesterol
  • Having a family history of heart problems

For patients with these risk factors, routine stress tests are a useful way to monitor changes in the heart. This can help physicians detect problems as they develop, letting them plan treatment to address the issue before it gets worse.
